These Terms & Conditions govern participation in the Graceful Pathways Client & Caregiver Referral Incentive Program (“Program”). By submitting a referral, participants agree to the following:
1. Eligibility
- The Program is open to individuals aged 18 or older.
- Referrals may include:
- Potential clients who may benefit from home care services.
- Potential caregivers who meet Graceful Pathways’ hiring standards.
- Employees of Graceful Pathways may participate unless otherwise restricted by internal policy.
2. Qualified Referrals
A referral is considered “qualified” when:
Client Referrals
- The referred individual completes an intake process and becomes an active client receiving services through Graceful Pathways.
Caregiver Referrals
- The referred caregiver completes the hiring process, meets all onboarding requirements, and remains in good standing for the minimum employment period defined by Graceful Pathways (typically 30 days unless otherwise stated).
Graceful Pathways reserves the right to determine whether a referral meets qualification criteria.
3. Reward Structure
Participants may earn rewards based on the number of qualified referrals they submit. Reward tiers may include, but are not limited to:
- Smart Device Incentives & Other Hard Premiums
- Discount Savings Certificates, Vouchers & Incentives
- Dining & Restaurant Certificates, Vouchers & Incentives
- Vacation Certificates & Incentives
- Cruise Certificates & Incentives
Rewards are subject to availability and may be substituted with items of equal or greater value at the discretion of Graceful Pathways.
4. Claiming Rewards
- Participants will be notified when a referral qualifies for a reward.
- Rewards must be claimed within 30 days of notification.
- If a reward is not claimed within this timeframe, it will be forfeited.
5. Reward Redemption
- Once a reward is claimed, participants have up to one (1) year to redeem the reward.
- Redemption instructions will be provided at the time the reward is issued.
- Unredeemed rewards after one year will expire and cannot be reinstated.
6. Limitations & Restrictions
- Rewards have no cash value and may not be exchanged for cash or credit.
- Rewards may not be transferred, sold, or bartered.
- Participants are responsible for any taxes, fees, or additional costs associated with reward redemption.
- Graceful Pathways is not responsible for lost, stolen, or expired rewards.
7. Program Modifications
Graceful Pathways reserves the right to modify, suspend, or terminate the Program at any time without prior notice. Any changes will not affect rewards already earned but not yet claimed.
8. Fraud Prevention
Graceful Pathways may disqualify participants for:
- Submitting false or misleading referral information
- Attempting to manipulate or abuse the Program
- Violating any Program rules or company policies
All decisions regarding Program eligibility and reward issuance are final.
9. Privacy
Referral information will be handled in accordance with Graceful Pathways’ privacy practices. By submitting a referral, participants confirm they have permission to share the referred individual’s contact information.
